The Alabama State Department of Education requires that every student, age 11 or older, provide documentation of a Tdap vaccine. Parents/Guardians must provide an updated Certificate of Immunization (COI) showing the Tdap vaccine before your 6th-grade student can pick up his/her 2024-2025 Athens Middle School schedule. Certificates can be dropped off at the AMS Main Office on Mondays - Thursdays from 8:00 AM - 4:00 PM. The deadline to turn in the Certificate of Immunization is July 25, 2024.

Please note:

  • All COIs must have the Alabama Department of Public Health seal.
  • No vaccine forecast or history is acceptable.
  • The COI cannot have handwritten information anywhere on the form. 
  • Alternate immunization forms and pharmacy printouts are not acceptable.
  • If your student turns 11 after the start of the school year, the COI must be updated and submitted as soon as they turn 11.
